Viewing the Help Plan in App - Care Worker & Member
How Care Workers and Members access a view of the Help Plan
We have a philosophy that Members should only tell their story once. Their story, goals, and, expectations are captured in a living document known as the Help Plan. It should outline what the care provider is doing for the Member, and what they are doing with the Member, as well as anything they need to be aware of to deliver great outcomes for the Member.
Member App
To see the Member's Help Plan from the Care Worker App:

Go to the People tab

Go to your or your loved ones name

Go to Help Plan
Care Worker (Helper) App
To see the Member's Help Plan from the Helper App:

Go to the "People" tab.

Select the Member you'd like to view the Help Plan of.

Select "Help Plan" from the menu.
